ダイエットをやめて自由になる: 摂食障害と薬・SSRIについて

摂食障害の治療において多く処方されている薬について書いています。
可能な限りの情報は調べましたが、私自身は特別な専門知識のない人間です。
その点をご理解いただいて、
この情報を必要とされる方のみ以下を読んでくださるように

はじめに、
私自身は摂食障害と薬を単純に結びつけること
（日本の風邪には、ルルが効く♪みたいな感じでの短絡的な結びつき）
そのものは納得いかない、と思ってる立場です。

個人的な経験を顧みても摂食障害を作り出す要素、というのは
「個人的な脳内物質のバランス」だけでは説明つかないだろう、
という気がしてるから、なんですが。

もちろん飲まない方がいい、と思ってるわけでもなく
飲んだ方がいい人もいるだろうし、
飲まなければならない人もいるだろうし、
飲まない方がいい人もいるかもしれない、
というふうに考えてます。
「摂食障害」に対しては
最大限補助的な役割を果たすにとどまるのではないかな、と。

が、いずれにせよ、薬に関する知識は欲しい。
いかなる理由で「それら」が摂食障害に対して処方されるのか知りたい、
あるいはどういった利益のために、副作用を我慢しなければならないのか知りたい
あるいはどういう選択肢を持ちうるのか知りたい、
という動機で、少し調べてみました。

調べてみると
「抗うつ剤（三環系、SSRI、SNRI）」
「マイナートランキライザー（睡眠薬、抗不安剤）」
「メジャートランキライザー（統合失調症用の薬）」
「その他（食欲抑制剤、ドグマチール、麻薬拮抗剤（？）、抗てんかん薬）」
など、多岐にわたり利用されています。
（※麻薬拮抗剤についてはときどきちらほら文献に載っていて、
どうやら、一時期摂食障害に効くのではないか、という論文も
多少あったようではあるのですが、その後はあまり研究されていないようです。）

まずは比較的新しく、そして摂食障害の薬としてはかなり注目されている
SSRIからです。

摂食障害治療に関する新しい文献を読むと
「今後SSRIが摂食障害治療薬の主流となるだろう」
というニュアンスのものが多いように感じます。

SSRIが希望を持たれている理由は
・従来の抗鬱剤に比べて副作用が少ないといわれていること
・従来の抗鬱剤は太るものが多かったことに比べて、肥満の副作用が少ないとされること

ではSSRIって何か。
SSRI（Selective Serotonin Reuptake Inhibitors=選択的セロトニン再取り込み阻害薬）
日本で使われている有名どころは、
ルボックス、デプロメール、パキシル、ジェイゾロフトなど。
主にうつ病や強迫性障害、社会不安障害に対して処方される薬ですね。
日本で認可されたのは1999年から、というかなり新しいもの。

飲んでる人は説明されていることと思いますが
効果が出るまで二週間くらいは掛かる、といわれてます。
でも副作用だけはすぐ出ます。
（↑これ、なんか不思議ですけどね。
副作用がある、ってことは身体に作用は起してる、ってことですから）
副作用としては、私が話しを聞いた中で多いのは吐き気、口が渇く、眠気です。

「選択的セロトニン再取り込み阻害薬」
これ読んで意味分かったらスゴイですよね。
私まったくわかりませんでした^^
「選択的」に「セロトニン」の「再取り込み」を「阻害」する「薬」です。
・・・って分割しただけで全然説明になってないですけどね。

脳の中には色々な化学物質があって
・ノルアドレナリン
・セロトニン
・ドーパミン
などが、少なくてもあるわけです。
あんまり細かいことはまだ分かってないんですが。

セロトニンというのは、どうやら
ドーパミン（喜び、快楽を伝達する物質）、
ノルアドレナリン（恐れ、驚きを伝達する物質）
などの情報をコントロールし、
精神を安定させる作用があるらしい、ということなんですね。
で、うつになる人というのは
脳内でこの「セロトニン」というのが凄く減ってるんじゃないか、
というのがいわゆる「セロトニン仮説」。
これも正直あんまり分かってないそうですけど。

では、このせっかく放出された「セロトニン」の中に
働かずにもとの場所に戻って再吸収されてしまうのがいるのを、
戻さないでちゃんと脳内のしかるべきところで働くようにしよう、というのがSSRIです。
シナプスに「再取り込み」されてしまうのを
「選択的」に「阻害」してセロトニンの再利用をしてやれば
脳内のセロトニンの連絡が良くなるではないか、ということ。

（ちなみに良く似た名前のSNRIというのは、
セロトニンとノルアドレナリンの両方に作用するものです。）

副作用に吐き気や食欲不振、便秘などが起こりやすいのも
「セロトニンに作用」するお陰です。
情報の伝達にセロトニンを使う細胞というのは脳内だけでなく
身体中にあるんですが、消化器官にとくに多く
90パーセントは腸に分布するのだそうです。
当然ですけど、副作用にもちゃんと理由があることに感心します。

副作用が少ないだろう、といわれるゆえんは
この「セロトニン」に特化して、「選択的」に働くからである、
他の脳内物質はあまりいじらないからであろう、
というふうに言われているわけです。
（本当に副作用が少ないのか、というところについては実は異説もあります。）

ちなみに、なぜありがたくない「副作用」はすぐ出るのに、
ありがたい「効果」はなかなかでないのか、といえば、
どうも、脳内にはこの「セロトニン」を
「もう作らなくていいよ」ということを伝える物質もあるのではないか、
SSRIというのはその「もう作らなくてもいいよ」連絡網にも働きかけて
「もう作らなくてもいいよ」とは言わなないように調整するのだけども
その「調整」に二週間くらいかかっているのではないか、
というようなことを言われているそうです。
なるほど。

日本で認可されているSSRI
物質名	薬品名
フルボキサミン	デプロメール、ルボックス
パロキセチン	パキシル
セルトラリン	ジェイゾロフト
このほかアメリカで最も有名なものがフルオキセチン（プロザック）日本では承認申請中であるらしいですが、現在未承認

アメリカ食品医薬品局FDA（医薬品の許可などを行う機関。
日本では厚生労働省がやってます）では、
このSSRIを「摂食障害に効果あり」としています。
いかなる根拠によってこれを判断したのかまではちょっと調べられませんでしたが、
フルオキセチン（商品名：プロザック/日本未承認）では
多施設で偽薬との比較研究が行われており、
高用量の処方で明らかに効果的だった、とのことです。

最近のエントリー

ちょうど1年前の今頃からデプロメール（ＳＳＲＩ）を飲んでいますが、副作用には参りました。吐き気が2週間ほど続いて、なれたところで１錠から２錠に増やしたところ、気分が悪くなりジャスコで失神してしまいました。怖くなりすぐに１錠に戻しました。
でも、過食の量、回数とも、減ってきていると思います。

Do you think we'll see the day when LCD monitors are replaced by holographic displays? Screens that literally appear out of thin air in full color?

What do you think? Holographic technology is still fairly new but there are scientists out there who believe we've only begun to tap into it.

There are also people working on holographic based cloaking devices for use in combat. In 10 years we could be looking back on the technology of today and laughing.

So my sister is 10 years old, and has been addicted to board games lately. She asks me like 5 times a day to play the game Life with her or Apples To Apples. So it got me this idea to make this thread.

I would like to know what is your favorite board/card/dice/etc. game, and what was your favorite board/card/dice/etc. game as a child?

My favorite board game is Clue, cause it is not like other board games in my opinion and keeps me entertained. I loved Clue as a kid, and I still do. Other games I enjoy are Uno and Apples To Apples.

Have you ever been to a themed party where you dressed up accordingly?

Every year we have a themed party which includes costumes, food, music, backdrops to match.
Over the years we've had the following themes: China, Under the Sea, Colours, Out of Space, Countries, Black & White, Hawaii, History, Fairy Tales... Can't think of anymore.

Anyway they are real fun, seeing what everyone dressed up as and how they interpredted the theme idea.

However, like any other drug, wrongly using these tablets will result in unpleasant side effects. It is advised that people refrain from exceeding a 24hr window between their Viagra prescriptions for people suffering from HIV this frame is increased to be 48 hrs.
Exceeding the prescribed dosage of Viagra can result in some severe side effects. One such common example is that of Priapism. Priapism is a situation where a man a man is left with a prolonged erection for many hours. Contrary to public opinion, this effect is extremely painful for the bearer. The effects of Viagra remain in the body for as much as 5 hours and during this period possible side effects might reoccur.
